TfL Journey Planner
Planner for journeys in and around London. It allows you to specify places in several ways (e.g.: postcodes, stations). You can use the 'Advanced options' to choose transport type, maximum walking time, disability options etc.
Transport for London
The "parent site" for the Journey Planner (see above), this is an informative site on London's buses (road and river), tubes, trams and DLR. Also has information on other modes of transport such as coach services, walking and driving in London and travel by taxi.
Transportdirect
Features a door-to-door journey planner from any origin and destination in the UK covering rail, buses, & air. This site is still in development; it is best to check locally (contact details are included). Traveline does a similar job, linking to region-by-region databases.
